Community Health
We’re still Community Health Centers of the Rutland Region, but we’ve shortened our name to Community Health to make it easier to remember. Community Health is a group of seven comprehensive primary care centers and two dental practices providing care to approximately 75% of Rutland County residents. Community Health has locations in West Pawlet, Shoreham, Brandon, Castleton and multiple Rutland locations. All sites are designated Level 3 Patient Centered Medical Homes for meeting national standards for care coordination and on-going quality improvement. All offices are accepting new patients. We offer Express Care in Rutland and Castleton for minor illnesses and injuries, 8am to 8pm Monday-Sunday and walk-ins are welcome.
